If you experience challenges getting to clinic appointments due to unreliable transportation, difficulties with public transit, or other limitations, you can complete the intake form to request transportation support. Transportation is offered exclusively for clinic or hospital visits and can include round-trip services if needed.9:00 AM and 5:00 PM, excluding weekends and holidays.
CVSAF is a nonprofit organization operating independently from any hospital or clinic. The Transportation Assistance Program aims to enhance access to medical care for individuals living with sickle cell disease in our community. This policy outlines the process for requesting transportation to healthcare facilities, helping to ensure timely medical care and address transportation barriers.
To ensure availability, transportation requests must be submitted at least 48 hours in advance using the designated online platform or by contacting CVSAF's Lift for Life Program. Requests are accepted Monday through Friday 9 AM and 5 PM.
